The programme is 12-weeks long and aims to help you to make lifestyle changes in aid of weight management. The idea is that these changes are small and realistic for your own individual circumstances, the most important thing about them being that they must be sustainable for you, which your health coach will support you with.
Once we’ve received your referral, you’ll receive a welcome email and SMS with instructions on how to begin. You'll need to download the Liva app, where you’ll be automatically assigned a personal health coach. From the moment you join, you’ll start receiving daily learning content, designed for you to work through at your own pace.
You will learn a range of information including nutrition, physical activity, stress, weight management, and how to make changes that you can stick with long term. The information is designed to be suitable to everyone so you may disregard what is not relevant to you and your health coach can then provide tailored support to help you with applying any relevant topics to your own priorities and goals.
Your health coach is your partner in progress, but they need your input to guide you best. Activity shows them what's working, what needs adjustments, and how to tailor your programme best for you.
The more you track goals, habits, and metrics, the clearer your roadmap to success becomes. Regular updates to your coach will help you to identify patterns, celebrate wins, and stay motivated.
Within the first few days, your health coach will introduce themselves via a message in the Liva app, and from then on you’ll receive regular support with weekly messages throughout the 12-week programme.
Throughout the programme, you’ll also be prompted to log your weight at various stages—this is an important requirement for staying on this NHS-funded programme.
To keep your access to health coach messaging and appointments, it’s essential to actively participate by submitting weight updates, interacting with your coach, and engaging with the learning content. If weight updates are not submitted or participation drops, messaging features will be disabled, though you will still continue to receive learning content.
